Online Dental and Vision Card - Monday, May 18, 2009

Dental and Vision cards are now available to Members to print out from the Website. You will need to Login and under Member Information, click on "Your Dental and Vision card". This will bring you to the first page where you can click for a printable version that you can bring to your Dental and/or Vision Providers to assist in Benefit information or Claims submission.

Your local, in conjunction with the benefits office, has launched this new Web site, allowing members to securely view contributions and other benefit related information anytime of day or night, any day of the week!

After logging into the site please take a moment to change the system generated username and password provided to you in the welcome letter you received about this new Web site.

After you log in please click on your account information link in the upper right hand corner of the screen. Once there you have the option of changing a variety of account information for this Website.

Members of Sheet Metal Workers Union Local 104 now have the ability to access Fringe Benefit Information 24 hours a day, 7 days a week.

To Navigate this site:

Click on a menu item above to go to that specific page of the site.

If you are a participant in the Fringe Benefit Funds, you can use your User Name and Password to gain access to specific data pertaining to your benefits.

To Login:

Click the Login link in the top right of the screen. This will bring up the Login screen.
Enter your User Name and Password then click on the submit button.

If your login was successful you'll see a new menu item named 'Member Information'.

If your login was invalid a simple message indicating what is wrong (username or password) will be displayed.

If your login has expired, please contact your Fund Office or the Web Master.
